I am contracted with Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, and Medicaid, but it is recommended you verify your benefits before assuming our appointments would be covered. Do you plan to use insurance?

When you make an appointment and it is confirmed, you must immediately upload an image of the front and back of your insurance card into your patient portal, in the documents section. This must be done on a computer and not your mobile phone. This helps the submission process run smoothly. Do you agree to do this?

To help ensure optimal coverage for your visit, please upload any notes from other healthcare providers that list or document your diagnoses. This includes primary care practitioners, specialists, and pshychiatrists or therapist. You will upload these under the "documents" section in your patient portal. Please choose "yes" below to acknowledge you will, or choose "no" to let me know you dont have any official diagnoses.

Preferred Contact Method:

How ready are you to follow a clear plan for at least 4-6 months?

How did you hear about me?

Your appointment email and patient portal email may go to spam. Type "yes" below to acknowledge you will check your spam folder for next steps.
